Bursitis
Hip bursitis becomes painful when the bursae – small, fluid-filled cushions – become inflamed. These bursae help the hip glide smoothly by acting as shock absorbers and lubricators between muscles and bones.
Overuse is the most common cause, but a fall or direct impact can trigger the inflammation as well. Pain usually worsens with movement, and many people can’t lie on the affected side. Running, climbing stairs, and even getting out of a car often become challenging.
The pain may radiate down the leg to the knee, and this condition tends to affect middle-aged women more frequently.
Muscle Strains
More than 60% of hip pain is caused by soft-tissue strains. These occur when the muscles or the muscle–tendon junction become overstretched or torn. Activities like running, jumping, twisting, or sudden movement can trigger them.
Symptoms typically begin with sharp pain, followed by increasing soreness, stiffness, weakness, and limited range of motion. Without proper treatment, these strains can become chronic and easily re-injured.
Hip Pointer
A hip pointer is a direct trauma injury – usually from a fall or impact – that bruises the muscles attached to the hip bone. Even the abdominal muscles may hurt or spasm, especially when turning toward the injured side.
Glute & Hip Flexor Strains
Strains of the lower gluteus maximus or hip flexors often arise from repetitive movements or forced stretching. Over time, pain increases and the muscles feel extremely tight. When one injured muscle can’t function properly, other surrounding muscles begin compensating – creating a predictable cascade of pain and dysfunction.
The iliopsoas, in particular, is one of the most frequently injured muscle groups in the hip and is often weakened by pelvic tilt or prolonged sitting.
